MedicalMex is launching a new informational and support service for internationally trained physicians interested in working in Texas under the new Texas House Bill 2038 (HB2038). This program is especially focused on serving doctors licensed in Mexico, though it is open to candidates from any country who meet the general eligibility criteria.

What is Texas HB2038?

Texas HB2038, also referred to as the DOCTOR Act, was signed into law in June 2025 and takes effect on September 1, 2025. It creates a new pathway for foreign-trained physicians and recent medical graduates to receive provisional or limited licenses to practice medicine in Texas.

This law was introduced in response to significant physician shortages in Texas, especially in rural and medically underserved communities. It acknowledges the skills of doctors trained outside the United States and offers a structured legal framework for them to contribute to the Texas healthcare system.

HB2038 is expected to be a game-changing opportunity for qualified international physicians, particularly from neighboring Mexico, to bring their talents into clinical practice in Texas.

Who is this Program For?

HB2038 is designed to serve:

  • Foreign-trained physicians with a valid medical license from their country of origin (such as Mexico)

  • Physicians who have passed or are preparing to take the USMLE Step 1 and Step 2 exams

  • Recent medical graduates who have not matched into a U.S. residency but meet educational and examination standards

  • Doctors with or without current U.S. work authorization (for those without it, MedicalMex will assist in obtaining H-1B or TN visas)

  • Physicians who are willing to work in supervised, facility-based healthcare settings in Texas

When Will This Be Available?

Texas HB2038 goes into effect on September 1, 2025. However, as of today, the Texas Medical Board (TMB) is still drafting final rules and licensing procedures.

Applications for provisional licensure are not yet open. MedicalMex is currently gathering inquiries and building a candidate and employer database so we can act quickly once the rules are finalized.

Based on the current language of HB2038, a foreign-trained physician applicant is expected to meet the following criteria:

  • Hold a medical degree (MD or equivalent) from an ECFMG-eligible or internationally recognized medical school

  • Have an active and unrestricted medical license in good standing from their home country

  • Have completed the USMLE Step 1 and Step 2 exams (or be in preparation)

  • Pass the Texas Medical Jurisprudence Exam

  • Demonstrate English proficiency

  • Be legally authorized to work in the United States or be eligible for visa sponsorship

  • Receive a job offer from a qualified Texas-based healthcare provider such as a hospital, urgent care clinic, or group medical practice affiliated with an ACGME or AOA-accredited residency program

If an applicant does not yet have U.S. work authorization, MedicalMex will assist in obtaining H-1B or TN visa sponsorship through its network of employers.

These are the average expected wages for similar roles in Texas today:

Physicians (General): The average annual salary for physicians in Texas is approximately $213,818, with a typical range between $138,862 and $254,304, depending on specialty, setting, and years of experience.

Primary Care / General Practice (Provisionally Licensed Roles): Entry-level or supervised roles—such as those available to unmatched U.S. graduates or internationally trained physicians holding a provisional license under HB2038—are likely to fall at the lower end of the physician pay scale. These positions typically offer between $120,000 and $180,000 per year, especially in primary care settings such as family medicine, internal medicine, and rural general practice.

Specialists: International physicians who are licensed to practice in their specialty (such as surgery, cardiology, gastroenterology, or OB/GYN) may command higher salaries in the range of $200,000 to $400,000+ per year. However, under HB2038, provisional status and supervised conditions may result in lower initial compensation compared to fully licensed peers, with room for growth upon obtaining full licensure.
